Abstract

The lecture presents information about one of the leading disorder of the menstrual function - amenorrhea. Amenorrhea is not a substantive diagnosis, but a symptom that indicates to changes in the system of reproduction due to various physiological or pathological causes. There are considered various forms of this disease, their etiological factors. There are presented classification of amenorrhea, the algorithm of examination in this type ofpathology, recommendations on treatment.

Uterine artery embolization in uterine fibroids patients of reproductive age and its role in shaping of autoimmune oophoritis

Sosnova E.A.

Abstract

In women of the reproductive age who have preserved the uterus after uterine artery embolization (UAE) and are interested in restoring fertility, the clinical evaluation of long-term results of this treatment method was executed with the use both of a questionnaire survey (n = 98), and determination of the status of ovarian reserve, ovaries, thyroid gland(n = 52). Performed retro- and prospective clinical-laboratory and instrumental methods of the investigation allowed to reveal a higher risk of the development of remote complications of UAE, in particular in the premature failure of ovarian function.

Morphological features of female gonadal dysgenesis and karyotype in patients with Shereshevsky-Turner syndrome

Kogan E.A., Ushakova O.E., Kur’yanova Y.N., Fayzullina N.M., Stupko O.K.

Abstract

The aim of the work - the study of the morphological and immunophenotypic variants of the structure gonads in gonadal dysgenesis (GD) and karyotypic characteristics of patients with the Shereshevsky-Turner syndrome (SHTS). There were investigated the tissue samples of the removed gonads and ovaries from 16 SHTS patients with GD and 11 patients from the control group without SSHT whose ovaries had been removed due to parovarian cysts. The morphological study was executed with immunohistochemical typing of gonadal tissue with the detection of expression: Ki-67, Vimentin, Desmin, Inhibin A, ER ( «DAKO»), AR ( «SpringBioscience»). The karyotype was investigated by means of cytogenetic method, whereas the presence of the SRY-gene - with aid of PCR-reaction in blood lymphocytes. As a result, there were established morphological variants of GD in SHTS: streak-gonads (8 patients), GD with ovarian stroma (6 patients), GD with the formation of the gonadoblastoma (2 patients). At that in the blood lymphocytes of SHTS patients there were discovered following karyotypes: 45Х, 45X/46XY, 45X/46XX, 45, Х, t (4, 19) (р14, q13.3), 45, Х[5]/46, Xi(X)(q10)[20], 45, Х[31]/46, Xi(X)(q10)[19], 46Xpsui(Y)(q10)[6]/45, Х[3]/46 XY[20]. There were revealed statistically significant correlations between karyotypes of blood lymphocytes and immunophenotypes of cell of the genital cord stroma, fibroblasts and GD germinal cells. Markers directly relating to the survival of follicles: inhibin-a, AR, ER were of the greatest importance for the development of someone or other morphological variant. GD in patients with ovarian stroma were differed in the greatest expression of ER (p < 0.05). SRY-gene was detected in the 1 patient with the lack of Y-chromosome. In two patients gonadoblastoma was found in GD without the presence of the SRY-gene in blood lymphocytes. The data obtained testify to a variety of morphologic and immunophenotypic variants of GD in SHTS, which are not always correlated with the karyotype of blood lymphocytes ofpatients that may partly reflect a possible discrepancy between the karyotypeofblood lymphocytes and DG tissue due to gene mosaicism.

Kaptilnyy V.A.

Abstract

In the article there is presented the technique of taking ecto- and endocervical scrapings for the cytological method of research. There are described in details the various techniques used in routine clinical practice. Particular attention is paid to the quality of the collecting cytological sample - technique of taking samples of cellular material. There is described the preparation to the study, readings and research goals. The accompanying photos illustrate in detail all the stages of the cytological screening of the uterine cervix.

Vozmozhnosti magnitno-rezonansnoy tomografii v differentsial'noy diagnostike razlichnykh tipov miomy matki

Ageev M.B., Sidorova I.S.

Abstract

A total of 37 patients of reproductive age with uterine myomas were examined and treated; MRT diagnosis was carried outin the standard and specifying modes. Biochemical characteristics of various types of myoma nodes were studied by 1H-MPS proton spectroscopy and dynamic contrast amplification. Tomograms of 37 women (58 nodes) and the morphology of 58 histological preparations mmpared. The results of this analysis demonstrated that complex MRT precisely differentiated the major histological types of uterine.

Effektivnost' metodov organosokhranyayushchego lecheniya miomy matki

Ageev M.B., Sidorova I.S.

Abstract

A total of 31 patients of reproductive age with uterine myomas were examined and treated after standard MRT diagnosis. Twenty (64.5%) patients received MRgFUS therapy (group 1). Embolization of the uterine arteries was carried out in 8 (25.8%) patients (group 2). Myomectomy was carried out in 3 (9.7%) patients (group 3). The treatment efficiency was evaluated 1, 3, 6, and 12 months by MRT, detailed ultrasonic examination, and quality of life score (PBAC tables (Higham, Janssen); Short-Form MacGill Pain Questionnaire; UFS-QoL Questionnaire).

Bezhenar' V.F., Fishman M.B., Gorbatenko N.V.

Abstract

According to WHO, in 2025 about 50% of women will be obese. There is a direct link between obesity and reproductive dysfunction of women. Conservative treatment of obesity is not always effective. Bariatric surgery has a positive effect on fertility in women who are obese.

Tactics of the surgical treatment of torsion of the SINGLE uterine appendages in nulliparous women

Ishchenko A.I., Ishchenko A.A., Bakhvalova A.A., Devitskiy A.A., Maksimov A.I.

Abstract

There are considered two clinical cases of torsion of the single uterine appendages in patients of reproductive age without pregnancies in a history. Both patients previously were undergone to adnexectomy due to adnexal torsion. After clinical and laboratory examination and intraoperative endoscopic visualization of the pelvic organs it was decided advisory to limit the amount of surgical intervention by means of unwinding the uterus by laparoscopic method. Taking into account the adnexal torsion on the opposite side (with subsequent adnexectomy) in a history the main stage of the surgical treatment was supplemented by ovariopexy to prevent disease recurrence. Postoperative control for the restoration of blood supply to the ovarian tissues includes programmed laparoscopic examination, monochrome ultrasound scanning, color Doppler mapping (CDM), the study of blood levels of sex hormones, a common clinical and biochemical analyzes of blood, coagulogram.
