This review presents current Russian and English studies on this topic. The key advantage of microRNAs as potential biomarkers is their high stability in biological fluids (blood, urine, amniotic fluid, and cervical mucus), tissue specificity, and the possibility of non-invasive detection. In recent years, various studies have been conducted to identify miRNAs as biomarkers in the blood plasma of women with endometriosis. The complex intracellular interactions between signaling pathways suggest that microRNAs may be involved in the pathophysiology of endometriosis, because they regulate many processes, including apoptosis and cell proliferation. The studies reviewed show that changes in microRNA expression are associated with several pregnancy complications, including uteroplacental-fetal blood flow disorders and preeclampsia. Current evidence suggests that microRNA dysregulation is one of the central molecular mechanisms of cervical insufficiency, and the results of pilot studies support the promise of mRNA technology for pathogenetic correction of chronic placental insufficiency. Promising future research directions include large-scale validation of miRNA biomarkers, development of safe methods for local modulation, and in-depth investigation of the role of exosomes in disease pathogenesis.</p>
</p></trans-abstract><kwd-group xml:lang="en"><kwd>endometriosis</kwd><kwd>infertility</kwd><kwd>miscarriage</kwd><kwd>cervical incompetence</kwd><kwd>intrauterine synechiae</kwd><kwd>microRNAs</kwd><kwd>review</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>эндометриоз</kwd><kwd>бесплодие</kwd><kwd>невынашивание</kwd><kwd>истмико-цервикальная недостаточность</kwd><kwd>внутриматочные синехии</kwd><kwd>микроРНК</kwd><kwd>обзор</kwd></kwd-group><kwd-group xml:lang="zh"><kwd>子宫内膜异位症</kwd><kwd>不孕症</kwd><kwd>流产</kwd><kwd>宫颈机能不全</kwd><kwd>宫腔粘连</kwd><kwd>微RNA</kwd><kwd>综述</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Nazri HM, Greaves E, Quenby S, et al.
