After receiving a histological conclusion on the presence of endometrial proliferation in the biopsy, the patients underwent drug therapy with didrogesterone for 6 months. 3 groups were formed: group 1 included 32 patients who planned pregnancy and received didrogesterone at 20 mg/day from the 16th to the 25th day of the menstrual cycle; Group 2 included 31 patients who were not interested in pregnancy, who received 20 mg/day of didrogesterone from day 5 to day 25, and group 3 included 30 patients who were interested in becoming pregnant in the near future, who received 20 mg/day of didrogesterone from day 11. on the 25th day of the cycle. 3 months after the end of therapy, the nature of the menstrual cycle was assessed in combination with ultrasound examination of the pelvic organs. Statistical analysis was performed using IBM® SPSS® Statistics 23.0.0.0. (p&lt;0.05).</p> <p><bold>Results:</bold> 3 months after the end of therapy, menstrual irregularities in combination with a thickened m-echo of more than 7 mm on the 5th-7th days of the menstrual cycle were detected in 12 (37.5%) patients from group 1, in 2 (6.4%) from group 2 and in 2 (6.6%) – from the 3rd group.</p> <p><bold>Conclusion.</bold> The prolonged cyclic didrogesterone therapy regimen from the 5th to the 25th days of the cycle and the cyclic regimen from the 11th to the 25th days of the cycle demonstrated a statistically significant superiority in the effectiveness of the treatment of AUB in the reproductive period caused by ovulatory dysfunction compared with the didrogesterone regimen from the 16th to the 25th-th days of the cycle.</p></abstract><trans-abstract xml:lang="ru"><p>Аномальные маточные кровотечения (АМК) часто встречаются в репродуктивном периоде и, несмотря на проводимое лечение, наблюдается значительное число рецидивов АМК, ассоциированных с овуляторной дисфункцией, что сопровождается высоким риском бесплодия и развития гиперплазии эндометрия (ГЭ).</p> <p><bold>Цель. </bold>Сравнение эффективности различных вариантов циклического режима терапии дидрогестероном для профилактики рецидивов аномальных маточных кровотечений, обусловленных овуляторной дисфункцией в репродуктивном периоде.</p> <p><bold>Материалы и методы</bold>. Обследованы 93 пациентки в возрасте 27-44 лет с аномальными маточными кровотечениями на фоне овуляторной дисфункции. После получения гистологического заключения о наличии пролиферации эндометрия в биоптате, пациенткам проведена медикаментозная терапия дидрогестероном в течение 6 месяцев. Сформированы 3 группы: в группу 1 вошли 32 пациентки, планировавшие беременность и получавшие дидрогестерон по 20 мг/сут с 16-го по 25-й день менструального цикла; в группу 2 – пациентки, не заинтересованные в беременности, в количестве 31 чел., получавшие дидрогестерон по 20мг/сут с 5-го по 25-й день, в 3-ю группу – 30 пациенток, заинтересованных в наступлении беременности в ближайшее время, получавшие дидрогестерон по 20 мг/сут с 11-го по 25-й день цикла. Спустя 3 мес после окончания терапии проводили оценку характера менструального цикла в сочетании с данными ультразвукового исследования органов малого таза. Статистический анализ проводили с использованием IBM® SPSS® Statistics 23.0.0.0. (р&lt;0,05).</p> <p><bold>Результаты:</bold> через 3 месяца после окончания терапии дидрогестероном нарушения менструального цикла в сочетании с утолщенным м-эхо более 7 мм на 5-7-й дни менструального цикла были выявлены у 12 ( 37,5%) пациенток из 1-ой группы, у 2 (6,4%) – из 2-ой группы и у 2(6,6%) – из 3-й группы.</p> <p><bold>Заключение.</bold> Пролонгированный циклический режим терапии дидрогестероном – с 5-го по 25-й дни цикла и циклический режим – с 11-го по 25-й дни цикла демонстрировали статистически значимое превосходство в эффективности лечения АМК в репродуктивном периоде, вызванных овуляторной дисфункцией, по сравнению с режимом приема дидрогестерона с 16-го по 25-й дни цикла.</p></trans-abstract><trans-abstract xml:lang="zh"><p/></trans-abstract><kwd-group xml:lang="en"><kwd>abnormal uterine bleeding</kwd><kwd>reproductive period</kwd><kwd>ovulatory dysfunction</kwd><kwd>didrogesterone</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>аномальное маточное кровотечение</kwd><kwd>репродуктивный период</kwd><kwd>овуляторная дисфункция</kwd><kwd>дидрогестерон</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Munro MG.
