Managing these patients is challenging, necessitating a balance between effective control of rheumatic disease activity for a favorable pregnancy outcome and minimizing risks to the fetus.</p> <p>This paper summarizes modern approaches to preconception care, pregnancy management, safe therapy selection, and monitoring of complications in patients with rheumatic diseases, primarily systemic lupus erythematosus and rheumatoid arthritis. The paper reviews relevant guidelines, including the updated 2024 EULAR, data from national and international registries, and findings of cohort and retrospective studies on pregnancy outcomes in patients with rheumatic diseases.</p> <p>Rheumatic disease activity during conception and gestation is a key predictor of adverse outcomes such as pre-eclampsia, fetal growth retardation, preterm delivery, and congenital abnormalities. Achieving sustained remission 6–12 months before pregnancy considerably increases the likelihood of having a healthy child. The therapeutic strategy is based on the personalized selection of drugs that are suitable for pregnancy and lactation. Hydroxychloroquine is essential for the treatment of systemic lupus erythematosus owing to its immunomodulatory and antithrombotic properties, established safety profile, and positive impact on outcomes. Tumor necrosis factor inhibitors can be used in rheumatoid arthritis until late pregnancy or in patients with high disease activity. Methotrexate, mycophenolate mofetil, and cyclophosphamide are strictly contraindicated during pregnancy and must be discontinued at the planning stage. Effective disease control requires multidisciplinary collaboration between a rheumatologist and an obstetrician-gynecologist at all stages, from preconception counseling and follow-up during pregnancy to postpartum care.</p> <p>Thus, favorable pregnancy outcomes in patients with rheumatic diseases are possible with careful pregnancy planning when in remission, safe and effective maintenance therapy, and continuous multidisciplinary monitoring. Adherence to these principles reduces risks for both mother and fetus.</p></abstract><trans-abstract xml:lang="ru"><p>Ревматические заболевания, такие как системная красная волчанка и ревматоидный артрит, часто дебютируют в репродуктивном возрасте, что определяет значительную частоту сочетания беременности и аутоиммунной патологии. Ведение таких пациенток представляет сложную задачу, требующую баланса между эффективным контролем активности ревматического заболевания для благоприятного исхода беременности и минимизацией рисков для плода.</p> <p>В данной статье представлены систематизированные современные подходы к прегравидарной подготовке, тактике ведения беременности, выбору безопасной терапии и мониторингу осложнений у пациенток с ревматическими заболеваниями, преимущественно с системной красной волчанкой и ревматоидным артритом. С этой целью проведён аналитический обзор актуальных клинических рекомендаций, включая обновлённые положения EULAR 2024 г., данных национальных и международных регистров, результатов когортных и ретроспективных исследований, посвящённых исходам беременности на фоне ревматических заболеваний.</p> <p>Установлено, что активность ревматических заболеваний в момент зачатия и во время гестации является ключевым предиктором неблагоприятных исходов: преэклампсии, задержки роста плода, преждевременных родов, врождённых аномалий. Достижение стойкой ремиссии за 6–12 мес. до беременности достоверно повышает шансы рождения здорового ребенка. Основой терапевтической стратегии является персонализированный выбор препаратов, совместимых с беременностью и лактацией. Гидроксихлорохин занимает центральное место в терапии системной красной волчанки благодаря своим иммуномодулирующим и антитромботическим свойствам, обладая доказанным профилем безопасности и положительным влиянием на исходы. Применение ингибиторов фактора некроза опухоли при ревматоидном артрите возможно до поздних сроков беременности при высокой активности заболевания. Категорически противопоказаны при беременности метотрексат, микофенолата мофетил и циклофосфамид, требующие отмены на этапе планирования. Эффективный контроль заболевания требует мультидисциплинарного взаимодействия ревматолога и акушера-гинеколога на всех этапах: от прегравидарного консультирования и динамического наблюдения во время беременности до ведения послеродового периода.</p> <p>Таким образом, успешные репродуктивные исходы у пациенток с ревматическими заболеваниями достижимы при условии тщательного планирования беременности на фоне ремиссии, применения безопасной и эффективной контролирующей терапии, а также непрерывного междисциплинарного мониторинга. Соблюдение данных принципов позволяет минимизировать риски как для матери, так и для плода.</p></trans-abstract><trans-abstract xml:lang="zh"><p>系统性红斑狼疮和类风湿关节炎等风湿性疾病通常在育龄期发病，这决定了妊娠与自身免疫性疾病并存的现象十分常见。这类患者的管理是一项复杂的任务，需要在有效控制风湿性疾病活动度以确保良好的妊娠结局与最大限度降低胎儿风险之间寻求平衡。</p> <p>本文介绍了针对风湿性疾病患者（主要是系统性红斑狼疮和类风湿关节炎）在孕前准备、妊娠管理策略、安全治疗选择以及并发症监测方面的系统化现代方法。为此，本文对现行临床指南进行了分析性综述，包括 2024 年更新的 EULAR 指南、国家和国际登记库数据，以及关于风湿性疾病背景下妊娠结局的队列研究和回顾性研究结果。</p> <p>研究表明，受孕时及妊娠期间的风湿性疾病活动度是导致不良结局（如子痫前期、胎儿生长受限、早产和先天畸形）的关键预测因素。在怀孕前 6-12 个月达到持续缓解可显著提高生育健康婴儿的几率。治疗策略的基础是个体化选择与妊娠和哺乳期兼容的药物。羟氯喹因其免疫调节和抗血栓特性，在系统性红斑狼疮的治疗中占据核心地位，并具有经证实的安全性以及对妊娠结局的积极影响。对于类风湿关节炎，若疾病活动度较高，可在妊娠晚期前使用肿瘤坏死因子抑制剂。甲氨蝶呤、霉酚酸酯和环磷酰胺在妊娠期间属于绝对禁忌，必须在备孕阶段停药。有效控制疾病需要风湿科医生与妇产科医生在各个阶段进行多学科协作：从孕前咨询、孕期动态监测到产后管理。</p> <p>因此，在病情缓解期进行周密的妊娠规划、采用安全有效的控制治疗以及进行持续的多学科监测，风湿性疾病患者是可以获得成功的妊娠结局的。遵循这些原则有助于将母亲和胎儿的风险降至最低。</p></trans-abstract><kwd-group xml:lang="en"><kwd>rheumatic disease</kwd><kwd>pathology</kwd><kwd>pregnancy</kwd><kwd>systemic lupus erythematosus</kwd><kwd>rheumatoid arthritis</kwd><kwd>treatment</kwd><kwd>review</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>ревматическое заболевание</kwd><kwd>патология</kwd><kwd>беременность</kwd><kwd>системная красная волчанка</kwd><kwd>ревматоидный артрит</kwd><kwd>лечение</kwd><kwd>обзор</kwd></kwd-group><kwd-group xml:lang="zh"><kwd>风湿性疾病</kwd><kwd>病理</kwd><kwd>妊娠</kwd><kwd>系统性红斑狼疮</kwd><kwd>类风湿关节炎</kwd><kwd>治疗</kwd><kwd>综述</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Pekareva NA.
