Despite significant progress in assisted reproductive technologies, the treatment of infertility associated with endometrial disorder and ovarian dysfunction remains relevant and requires the search for new therapeutic approaches.</p> <p>This review analyzes current data from Russian and international sources published over the past 5 years on the use of autologous platelet-rich plasma (PRP) as a promising therapeutic modality in assisted reproductive technology programs. The search for published data was conducted in PubMed, eLibrary, and Google Scholar databases. Analysis of the publications demonstrated that PRP therapy is clinically effective in treating thin endometrium, recurrent implantation failure, and premature ovarian insufficiency and may also improve the clinical pregnancy rate. However, the available studies are limited by small sample sizes and nonrandomized designs, precluding definitive conclusions regarding the efficacy and safety of PRP therapy. Additionally, different routes of administration, injected plasma volumes, and platelet concentrations are currently in use. However, their efficacy has not been compared against one another but only against control groups receiving conservative therapy. This technology requires further investigation in randomized clinical trials to develop standardized protocols and evaluate long-term outcomes.</p></abstract><trans-abstract xml:lang="ru"><p>По данным Всемирной организации здравоохранения, бесплодие затрагивает 17,5% взрослого населения мира и остаётся серьёзной медико-социальной проблемой. Несмотря на существенный прогресс в сфере вспомогательных репродуктивных технологий, терапия бесплодия, обусловленного патологией эндометрия и овариальной дисфункцией, является актуальной и требует поиска новых решений.</p> <p>В обзоре проанализированы современные данные из российских и зарубежных источников за последние 5 лет о применении аутологичной тромбоцитарно-обогащённой плазмы (PRP) как перспективного метода терапии в рамках программ вспомогательных репродуктивных технологий. Поиск литературы производили в базах данных PubMed, eLibrary и Google Scholar. Анализ публикаций показал, что PRP-терапия демонстрирует клиническую эффективность в лечении синдрома тонкого эндометрия, повторных неудачных имплантаций и преждевременной недостаточности яичников и, возможно, влияет на частоту наступления клинической беременности. Однако имеющиеся исследования ограничены малой выборкой и нерандомизированным дизайном, что не позволяет сделать выводы об эффективности и безопасности PRP-терапии. Также на данный момент существуют разнообразные формы введения препарата, объём введённой плазмы и концентрация тромбоцитов в сыворотке, эффективность которых не сравнивают между собой, а только сопоставляют с группой контроля, получающей консервативную терапию.
