At present, the effects of menopausal hormone therapy and phytoestrogen therapy on antioxidant homeostasis are actively discussed. Available data are inconsistent, indicating that estrogen may exert both pro-oxidant and antioxidant effects, which makes this issue particularly relevant.</p> <p><bold>AIM:</bold> To investigate the effects of menopausal hormone therapy and phytoestrogens (soy isoflavones and resveratrol) on the oxidant/antioxidant profile of blood plasma in peri- and postmenopausal women.</p> <p><bold>METHODS: </bold>The study included 92 patients with climacteric syndrome who were divided into four groups: group 1, menopausal hormone therapy (<italic>n</italic> = 25); group 2, soy isoflavone therapy (<italic>n</italic> = 23); group 3, resveratrol therapy (<italic>n</italic> = 24); and a control group, no drug therapy (<italic>n</italic> = 20). The severity of climacteric syndrome was assessed using questionnaire-based scales before treatment and after 3 months. The antioxidant profile of blood plasma was analyzed using kinetic luminol-enhanced chemiluminescence.</p> <p><bold>RESULTS: </bold>The most pronounced reduction in climacteric syndrome severity was observed with menopausal hormone therapy using estradiol and dydrogesterone combination therapy. A positive clinical trend was also noted in the resveratrol group. Soy isoflavone therapy did not have a significant effect on the course of climacteric syndrome. Neither menopausal hormone therapy nor soy isoflavone therapy influenced antioxidant profile parameters. Resveratrol exerted a favorable effect on the thiol antioxidant system, and a correlation was identified between the status of the thiol antioxidant system and the severity of climacteric symptoms.</p> <p><bold>CONCLUSION:</bold> The findings demonstrate high clinical relevance and indicate the safety of menopausal hormone therapy and resveratrol with respect to the oxidant/antioxidant homeostasis of the blood, as well as their pronounced clinical efficacy. Further studies are needed to evaluate long-term outcomes.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование. </bold>Одним из важнейших последствий изменения уровня эстрогенов в пери- и постменопаузе является развитие оксидативного стресса, который играет одну из ключевых ролей в патогенезе возраст-ассоциированных заболеваний. На сегодняшний день активно обсуждается влияние менопаузальной гормональной терапии и терапии фитоэстрогенами на антиоксидантный гомеостаз. Имеются противоречивые данные, свидетельствующие о том, что эстроген может обладать как про-, так и антиоксидантными свойствами, что делает изучение этого вопроса особенно актуальным.</p> <p><bold>Цель.</bold> Изучить влияние менопаузальной гормональной терапии и фитоэстрогенов (изофлавонов сои и ресвератрола) на оксидантно-антиоксидантный профиль плазмы крови в пери- и постменопаузе.</p> <p><bold>Методы. </bold>В исследовании участвовали 92 пациентки с климактерическим синдромом, которых разделили на 4 группы: группа 1 — менопаузальная гормональная терапия (<italic>n</italic>=25), группа 2 — терапия изофлавонами сои (<italic>n</italic>=23), группа 3 — терапия ресвератролом (<italic>n</italic>=24), группа контроля — без медикаментозной терапии (<italic>n</italic>=20). До начала лечения и через 3 мес. оценивали степень выраженности климактерического синдрома по шкалам-опросникам и анализировали антиоксидантный профиль плазмы крови методом кинетической люминол-активированной хемилюминесценции.</p> <p><bold>Результаты.</bold> В уменьшении степени выраженности климактерического синдрома наиболее эффективной оказалась менопаузальная гормональная терапия комбинированными препаратами эстрадиола и дидрогестерона, положительная динамика также прослеживалась в группе терапии ресвератролом. Терапия изофлавонами сои не оказала значимого влияния на течение климактерического синдрома. Как менопаузальная гормональная терапия, так и терапия изофлавонами сои не влияла на параметры антиоксидантного профиля. Ресвератрол оказывал благоприятное действие на состояние системы тиоловых антиоксидантов, при этом была обнаружена корреляция между состоянием системы тиоловых антиоксидантов и степенью выраженности климактерических симптомов.</p> <p><bold>Заключение.</bold> Полученные данные имеют высокую практическую ценность и свидетельствуют о безопасности менопаузальной гормональной терапии и ресвератрола в отношении оксидантно-антиоксидантного гомеостаза крови, а также об их выраженной клинической эффективности.
