For a long time, pregnancy in women with aortic coarctation was considered extremely high risk, and pregnancy termination was often recommended. Pregnancy in women with unrepaired aortic coarctation is frequently associated with various complications, including uncontrolled hypertension, preeclampsia, hypertensive crisis, congestive heart failure, stroke, and aortic rupture or dissection.</p> <p>At present, there are no clear guidelines or a unified approach to the management of pregnancy in women with aortic coarctation. Recent studies indicate that, with appropriate management, pregnancy may proceed without serious complications. The main reasons for hospitalization during pregnancy in these patients include heart failure, uncontrolled hypertension, and supraventricular arrhythmias. Therefore, strict blood pressure control appears to be a key component in the management of pregnant women with this condition. A clinical case describing pregnancy management and delivery in a patient with newly diagnosed aortic coarctation is also presented.</p></abstract><trans-abstract xml:lang="ru"><p>В данной статье представлен обзор этиологии, патогенеза, клинических проявлений и диагностики коарктации аорты, которая составляет 6–7% от всех врождённых пороков сердца, у беременных встречается с частотой 1 случай на 4500 родов. Долгое время беременность у женщин с коарктацией аорты считалась беременностью крайне высокого риска, пациентке нередко предлагали её прерывание. Беременность у женщин с неоперированной коарктацией аорты часто связана с различными осложнениями, среди которых неконтролируемая артериальная гипертензия, преэклампсия, гипертонический криз, застойная сердечная недостаточность, инсульт, разрыв и расслоение аорты.</p> <p>В настоящее время нет чётких рекомендаций и единого подхода к ведению беременности у женщин с коарктацией аорты. Исследования последних лет показывают, что при правильном подходе к ведению беременности она может протекать без серьёзных осложнений. Основной причиной госпитализации во время беременности у данных пациенток является сердечная недостаточность, неконтролируемая гипертензия и наджелудочковая аритмия. Именно поэтому строгий контроль артериального давления представляется целесообразным при ведении таких беременных. Также описан клинический случай ведения беременности и родоразрешения пациентки с впервые выявленной коарктацией аорты.</p></trans-abstract><trans-abstract xml:lang="zh"><p>本文综述了妊娠期主动脉缩窄的病因学、发病机制、临床表现及诊断特点。主动脉缩窄约占所有先天性心脏病的6–7%，其在妊娠中的发生率约为每4500例分娩中1例。长期以来，合并主动脉缩窄的妊娠被认为属于极高风险妊娠，临床实践中常建议终止妊娠。未接受手术治疗的主动脉缩窄孕妇，其妊娠过程往往伴随多种并发症，包括难以控制的动脉高血压、先兆子痫、高血压危象、充血性心力衰竭、脑卒中，以及主动脉破裂或夹层。</p> <p>目前，针对合并主动脉缩窄孕妇的妊娠管理尚缺乏统一的推荐意见和标准化处理策略。近年来的研究表明，在合理的妊娠管理条件下，此类妊娠可在无严重并发症的情况下完成。该类患者在妊娠期间住院的主要原因包括心力衰竭、动脉高血压控制不良以及室上性心律失常。因此，在此类孕妇的管理过程中，严格控制动脉血压具有重要意义。文中同时介绍了一例妊娠期首次诊断为主动脉缩窄患者的妊娠管理及分娩经过。</p></trans-abstract><kwd-group xml:lang="en"><kwd>aortic coarctation</kwd><kwd>congenital heart disease</kwd><kwd>pregnancy</kwd><kwd>hypertension</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>коарктация аорты</kwd><kwd>врождённый порок сердца</kwd><kwd>беременность</kwd><kwd>артериальная гипертензия</kwd></kwd-group><kwd-group xml:lang="zh"><kwd>主动脉缩窄</kwd><kwd>先天性心脏病</kwd><kwd>妊娠</kwd><kwd>动脉高血压</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Clinical recommendations of “Aortic coarctation”.
