Москвы «Центр медицинской профилактики Департамента здравоохраненя города Москвы»</institution></aff></aff-alternatives><pub-date date-type="pub" iso-8601-date="2020-12-27" publication-format="electronic"><day>27</day><month>12</month><year>2020</year></pub-date><volume>7</volume><issue>4</issue><issue-title xml:lang="en"/><issue-title xml:lang="ru"/><fpage>201</fpage><lpage>204</lpage><history><date date-type="received" iso-8601-date="2020-12-26"><day>26</day><month>12</month><year>2020</year></date><date date-type="accepted" iso-8601-date="2020-12-26"><day>26</day><month>12</month><year>2020</year></date></history><permissions><copyright-statement xml:lang="en">Copyright ©; 2020, Eco-Vector</copyright-statement><copyright-statement xml:lang="ru">Copyright ©; 2020, ООО "Эко-Вектор"</copyright-statement><copyright-year>2020</copyright-year><copyright-holder xml:lang="en">Eco-Vector</copyright-holder><copyright-holder xml:lang="ru">ООО "Эко-Вектор"</copyright-holder><ali:free_to_read xmlns:ali="http://www.niso.org/schemas/ali/1.0/"/></permissions><self-uri xlink:href="https://archivog.com/2313-8726/article/view/56954">https://archivog.com/2313-8726/article/view/56954</self-uri><abstract xml:lang="en"><p><bold><italic>The aim</italic></bold><italic> of the study is to assess the restoration of anatomical and functional integrity of the uterus and the blood flow in the uterine and ovarian vessels after laparoscopic and abdominal myomectomy.</italic></p> <p><bold><italic>Material and methods</italic></bold><italic>. 58 patients aged 36.2 ± 5.9 year old with uterine myoma underwent myomectomy: 31 patients underwent laparoscopic myomectomy and 27 patients underwent abdominal myomectomy.</italic></p> <p><italic>The selection of surgical approach didn´t depend on the size, the number and localization of uterine myomas and the patient´s concomitant pathology.</italic></p> <p><italic>Postoperative assessment was performed on day 5</italic><italic><sup>th</sup></italic><italic>–8</italic><italic><sup>th</sup></italic><italic> after surgery and in 1 and 6 months after myomectomy. 3D ultrasound examination with color Doppler mapping was used to assess the structure of the uterine scars after myomectomy and the blood flow in the uterine and ovarian arteries.</italic></p> <p><bold><italic>Results.</italic></bold><italic> No significant differences were found in the processes of reparation of the uterus and in the blood flow in the uterine and ovarian arteries after laparoscopic or abdominal myomectomy. The time, required for the patients´ reabilitation and for the total restoration of the uterine morphological structure was about 6 months and was similar in both groups.</italic></p> <p><bold><italic>Conclusion.</italic></bold><italic> Restoration of the anatomical and functional uterine integrity and the blood flow in the uterine and ovarian arteries after myomectomy doesn´t depend on the surgical approach if myomectomy is performed by experienced surgeon after the correct assessment of the clinical situation.</italic></p></abstract><trans-abstract xml:lang="ru"><p><bold><italic>Цель</italic></bold><italic> работы — оценить восстановление анатомической и функциональной целостности матки, а также кровообращения в сосудах матки и яичников после проведённой миомэктомии лапаротомным и лапароскопическим доступами.</italic></p> <p><bold><italic>Материал и методы.</italic></bold><italic> В исследовании участвовали 58 пациенток с миомой матки; всем выполнена миомэктомия (31 пациентке лапароскопическим доступом, 27 больным — лапаротомным). Выбор операционного доступа не зависел от размеров опухолевых узлов, их количества, локализации и сопутствующей соматической патологии. Средний возраст женщин составил 36,2 ± 5,9 года. Послеоперационное наблюдение проводили на 5–8-е сутки после операции, а также через 1 и 6 мес после проведённого лечения. Исследование структуры послеоперационных рубцов, а также кровотока в бассейне маточных и яичниковых артерий произведены в 3D-режиме с применением цветового допплеровского картирования.</italic></p> <p><bold><italic>Результаты.</italic></bold><italic> Проведённые исследования не выявили достоверных различий в репарации миометрия оперированных маток, а также изменений кровотока в маточных и яичниковых артериях после миомэктомии, выполненной с помощью двух разных (лапароскопического и лапаротомного) доступов. Сроки реабилитации пациенток и восстановления морфологической структуры матки также не имели значимых отличий и составили 6 мес для обеих исследуемых групп.</italic></p> <p><bold><italic>Заключение.</italic></bold><italic> Сроки и качество восстановления мышечной ткани матки, а также кровообращение в репродуктивных органах не зависят от доступа для хирургического вмешательства при адекватной квалификации хирурга и правильной оценке клинической ситуации.</italic></p></trans-abstract><kwd-group xml:lang="en"><kwd>myomectomy</kwd><kwd>blood circulation in uterine and ovarian vessels</kwd><kwd>uterine scar after myomectomy</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>миомэктомия</kwd><kwd>кровообращение в сосудах матки и яичников</kwd><kwd>рубец на матке после миомэктомии</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Dolinskiy AK, Tolibova GKh, Tral’ TG, Bezhenar’ VF.
